New technique helps orthopedic surgeon align bone break, avoid soft-tissue injury during surgery

Dr. David Silverberg, lead surgeon at the Las Vegas-based Silver State Orthopedics, discovered a creative way to help align ends of a broken bone to promote healing and mitigate tissue damage during surgery.
Silverberg was recently consulted to help repair a tibia that a young woman had broken in a ground-level fall. Silverberg said the case was fairly straightforward in terms of fracture pattern and fixation.
During fracture-care surgery, he said, bone ends must be aligned to promote healing and avoid deformity. In this case, attempts at aligning the bone proved unsatisfactory with traditional methods.
One option, he said, was to squeeze the bone ends together with a clamp to minimize the space between fracture fragments. This can be done, he said, by making two small skin incisions to grab the bone. But keeping the clamp’s pointed ends on the bone without slipping and hurting the soft tissues in between the clamp’s two points can be challenging.
Silverberg decided to place one end of the clamp on the bone as usual. This time, though, the other end of the clamp did not go through the skin, but compressed a folded towel lying on the skin. This squeezed the fracture together in near perfect alignment without the need to cut further or risk injuring the soft tissues.
The inserted rod across the fracture will maintain the fracture alignment until the bone heals.
